Title: LPN - Medical Assistant - Catalpa Health Location: WI-Appleton Catalpa Health has an opening for a LPN-Medical Assistant. This is a full-time position scheduled to work 40 hours a week. Hours: 7:30 AM - 4:30 PM (occasionally may work until 6:30 PM) Catalpa Health is a new joint venture between Affinity Health System, Children's Hospital of Wisconsin and ThedaCare that will provide a full continuum of outpatient mental health services to children and adolescence in the Fox Valley. Catalpa will be opening on November 1st. Job Duties: 1. Room patients- measure height, weight, pulse, and blood pressure. 2. Review immunizations. 3. Maintains medication refill line. 4. Review and prepare prescriptions for Physician signature. Process prescriptions for patient. 5. Ensures accuracy of electronic medical record related to patient care. 6. Answer patient calls-triage call and review with appropriate Physician/APNP. 7. Triage inpatient facilities request to coordinate patient care. 8. Provide triage and care coordination of client/provider calls to determine appropriate level of care. 9. Consult with medical staff when needed. 10. Refer to external resources when appropriate. 11. Monitors patient attendance and review with patient's provider for treatment non compliance. 12. Order labs and diagnostic testing. Monitors and process results with providers. 13. Gives scheduled outpatient injections 14. Maintain reading status for employee TB tests 15. Maintains orderly exam rooms. 16. Completes pre-authorization paperwork for medication request as needed 17. Perform other duties as assigned. * Minimum Associate degree in LPN, Medical Assistant or RN with experience in behavioral health required. * Maintain licensure with State of Wisconsin. Previous customer service experience preferred. * Adept in keyboarding skills. * Ability to work with a diverse patient population. * Self-starter with strong multi-tasking skills. * EPIC experience preferred. * Ability to transfer patients.
